Position: Social Worker - Discharge Planning

Exemption Status: Exempt

Schedule Details: Friday, On Call - Required; Thursday, Tuesday, Wednesday

Scheduled Hours: 3p-1130p

Shift: 2 - Evening Shift, 8 Hours (United States of America)

Hours: 32

Cost Center: 10030 - 4400 Social Services

Responsibilities:
  1. Assess patients and families, providing counseling and ensuring maximum participation in discharge and recovery plans.
  2. Identify system or discharge issues, develop discharge plans, and present options to patients.
  3. Assess potential cases of abuse: child, elderly, disabled, and domestic violence.
  4. Provide clinical services including biopsychosocial assessments, crisis intervention, treatment planning, and counseling related to medical conditions.
  5. Identify clinical resources and make referrals as needed.
  6. Collaborate with healthcare professionals to identify and resolve discharge or treatment barriers.
  7. Advocate for patients and families within the healthcare system.
  8. Document all evaluations, assessments, consultations, referrals, and dispositions to ensure regulatory compliance and optimal patient outcomes.
  9. Use DSM-IV-TR language for diagnostic evaluations and planning.
  10. Comply with departmental policies, participate in meetings and seminars, and adhere to safety regulations.
  11. Maintain professionalism, respect diversity, and ensure reliable attendance.
  12. Perform other related duties as required.
Qualifications:

Education and Licensure:

  • Master's Degree in Social Work.
  • Current Massachusetts Licensed Clinical Social Worker (LCSW) licensure.

Experience and Skills:

  • Psychosocial assessment, community resource knowledge, crisis intervention, discharge planning, and team collaboration skills.
  • Data analysis, excellent communication, and basic computer skills.
  • Preferred: 1-3 years of Social Work experience.
Physical Demands and Environment:

Primarily indoor work in an office; may include monitoring patients, requiring physical responses such as standing, walking, reaching, and lifting.
