Clinical Social Worker I/II – Outreach Role

Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II (CSW I/II) conducts in‑person outreach in single‑room occupancy (SRO) hotels throughout San Francisco’s Tenderloin, South of Market, and Mission neighborhoods.

Clients are formerly homeless adults supported by a city subsidy; the role focuses on engagement, clinical case management, linkage to mental‑health, substance‑use, and medical services, and building relationships to reduce eviction risk and maintain housing stability.

Responsibilities
  • Engage, assess, treat plan, and provide psychotherapy and crisis intervention to a caseload of formerly homeless clients.
  • Conduct 5150 assessments, crisis holds, and coordinate with acute hospitals and outpatient providers.
  • Advocate for services, collaborate with property management and support‑service staff, and communicate with families.
  • Document all interventions, assessments, and referrals in Epic and maintain records to meet Medi‑Cal and county requirements.
  • Participate in weekly clinical team meetings, clinical supervision, and ongoing training.
  • Perform physical duties requiring the ability to lift 50 lb and climb stairs.
Minimum Qualifications
  • Master’s degree in social work, psychology, or related field from an accredited university.
  • Registered or eligible for registration with the California Board of Behavioral Science as ACSW, IMFT, or equivalent.
  • Experience in community‑based case management, outreach, and working with clients who have severe mental‑health symptoms, substance use, and homelessness.
  • Proficiency with DSM‑V diagnostic criteria, harm‑reduction, trauma‑informed care, and anti‑racism practices.
  • Excellent communication, interpersonal, and teamwork skills across a culturally diverse setting.
  • Ability to work in a high‑stress, high‑volume environment, meet deadlines, and maintain detailed written records.
  • Physical ability to lift 50 lb and climb stairs.
  • Reliable attendance and background clearance.
Preferred Qualifications
  • Spanish language fluency.
  • Prior experience in a university‑centered clinical setting.
