Job summary

UCSF Health in San Francisco seeks a skilled mental health professional to join the Citywide Case Management Forensics Program. Job Description

The incumbent will be assigned to the Citywide Case Management Forensics Program. In this position, the incumbent will collaborate with criminal justice partners including Jail Behavior Health, NoVA, Behavioral Health Court, Mental Health Diversion Court, and SFGH Psychiatric Emergency Services in working with dually diagnosed adults in the community. The incumbent will provide comprehensive assessment, crisis intervention, case management, individual and group therapy, and substance abuse services. Clients will receive their mental health and substance abuse services at the Center or wherever they are located. The incumbent will assist clients in meeting their needs and will make referrals as appropriate. Our programs are part of the SFGH Department of Psychiatry’s comprehensive effort to treat high‑risk clients in the community and to reduce unnecessary use of institutional care.

Responsibilities
  • Service Provision: Provides ongoing mental health services, including psychotherapy (individual, group, and family), crisis intervention, case management, and outreach to a caseload of severely mentally ill clients; makes case terminations and discharge plans when clinically appropriate.
  • Service Planning, Consultation and Liaison: Develops comprehensive individual service plans; provides consultation to a multidisciplinary treatment team; facilitates treatment coordination among agencies and advocates for linkage to needed services; maintains liaison with campus and community programs; acts as liaison to inpatient units.
  • Intake/Assessment: Screens referrals for program criteria; conducts intake assessments, community functioning, and service needs; and responds to unscheduled walk‑in clients utilizing crisis intervention techniques and 5150 services.
  • Record Keeping and Administration: Maintains client records per California, City, and County requirements; records data in client charts; meets program productivity standards; completes billing and other documentation promptly; attends staff and clinical team meetings and participates in training sessions.
  • Education and Training: Provides education to the community regarding program services, client needs, and treatment; assists in planning and providing in‑service to staff; provides orientation and didactic training to interns; supervises student(s) and participates in student training program meetings as needed.
