Job summary

Qualifications

• Master's degree in Social Work is required.
• Active licensure or eligibility to obtain LISW/LPCC is preferred.

Responsibilities

• Conducts patient/family interviews, chart reviews, and interdisciplinary collaboration to assess biopsychosocial functioning.
• Develops care plans and discharge planning integrated with medical care.
• Coordinates with interdisciplinary staff and external partners to optimize patient care and resource use.
• Maintains thorough documentation in clinical records per standards and regulations.
• Provides community resource linkage and crisis intervention as needed.
