Qualifications

Responsibilities

• Conducts individualized patient assessments and prioritizes data collection.
• Develops and updates an individualized care plan with the care team.
• Performs treatments and nursing interventions per provider orders and plan of care.
• Evaluates patient responses and adjusts plans to improve outcomes.
• Provides patient and family education and supports shared decision-making.
• Collaborates with multidisciplinary team to ensure appropriate care and safety.
