Overview
The Clinical Resource RN is an experienced registered nurse who uses the nursing process (assess, plan, implement, evaluate) to provide clinical staffing support across high‑need Well Care Home Health regions. This role provides direct nursing services, case management support, and care coordination during vacancies, census fluctuations, and other organizational staffing needs. The RN is deployment‑ready and flexible to travel across assigned locations while maintaining continuity of care, high‑quality patient outcomes, regulatory compliance, and agency standards.
Primary Job Duties
- Completes initial, comprehensive, ongoing, and reassessment activities for patients according to patient needs, assigned program requirements, and location‑specific workflows.
- Develops, implements, and updates individualized plans of care in collaboration with physicians, patients, families/caregivers, and interdisciplinary teams.
- Provides direct nursing services and case management support in accordance with the plan of care, physician orders, and agency policies.
- Communicates with physicians, branch leadership, and interdisciplinary team members regarding patient needs, response to treatment, changes in condition, and required orders.
- Organizes and performs work effectively and efficiently.
- Maintains and adjusts schedule to support deployment needs, assignment timelines, and agency performance.

Quality of Work (30%)
- Utilization of Nursing Process (10%) – Uses assessment, planning, implementation, and evaluation to manage Home Health patient care. Provides thorough and timely assessments, completes reassessments and recertification activities, obtains physician orders, and implements care plans while adapting to branch processes.
- Care Coordination & Supervision (7%) – Manages assigned patient care responsibilities, coordinates care with interdisciplinary teams, supervises aides and LPNs, participates in interdisciplinary conferences, maintains caseload and workflow, and reports adverse changes to physicians, leadership, and risk management.
- Documentation & Communication (8%) – Documents patient eligibility, functional limitations, homebound status, hospice eligibility, symptom management, and all verbal orders accurately; completes clinical notes within agency time frames and records interdisciplinary communication.
- Team Contribution (5%) – Participates in the Clinical Resource Team, incorporates multidisciplinary recommendations, demonstrates willingness to travel and accept temporary assignments, embraces constructive criticism, and seeks improvements in workflows.

Job Specifications
- Education: Graduate of an accredited nursing program (AD, Diploma, or BSN).
- Licensure / Certifications: Current RN license in the state of practice; multi‑state licensure or ability to obtain licensure for assigned areas may be required. CPR certification required.
- Experience: Minimum two years RN experience, with Home Health experience required. Demonstrated ability to work independently, adapt to multiple clinical environments, manage patient care responsibilities, coordinate interdisciplinary care, and support high‑need locations during temporary assignments.
- Technical / Motor Skills: Hand/eye coordination for injections, computer and electronic medical record usage, operation of patient care equipment, and ability to transport patients.
- Interpersonal Skills: Ability to develop positive interactions with patients, families/caregivers, physicians, interdisciplinary team members, and staff.
- Physical Requirements: Must transfer and move objects weighing at least 50 pounds, frequently lift, push, move, and position patients; occasional reaching, stair climbing, and fine motor manipulation.
- Mental Abilities: Assess patient condition, formulate care plans, evaluate responses, teach recovery, prioritize duties, and learn new care skills.
- Sensory Requirements: Visual and auditory assessment for dressings, IVs, alarms, and communication with patients, families, and staff.
- Exposure to Hazards: Noise, blood‑borne pathogens, infectious diseases, needle puncture wounds, dangerous animals, traffic hazards, weather extremes, and potential personal safety threats during home visits.
- Hours of Work: Variable Monday‑Friday, weekends, holidays, on‑call; flexible schedule and temporary travel assignments required.
- Population Served: Adults and geriatrics.
- Transportation: Valid driver’s license, operational vehicle, ability to travel across assigned locations, and meet agency mileage, travel, and lodging arrangements.
