Job summary

Qualifications

Responsibilities

• Screen patients and identify eligibility for HealthNow program activities.
• Approach patients and explain HealthNow program processes/services; obtain consent and enroll participants when eligible.
• Coordinate point-of-care tests and discuss results with patients.
• Refer patients to appropriate linkage to care or community resources.
• Assist with data collection, entry, maintenance and reporting of study data; prepare protocol files for audits.
