Job summary

Ridgewood Rehab & Care Center is seeking an experienced MDS Coordinator to ensure accurate assessments, care planning, and regulatory compliance for residents receiving quality care.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• Knowledge of MDS, care planning, and long-term care regulations preferred.
• Strong organizational, analytical, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with state and federal regulations.
• Collaborate with nursing, therapy, dietary, social services, and other departments.
• Assist with developing and updating resident care plans.
• Monitor documentation to ensure accuracy and regulatory compliance.
• Support quality improvement initiatives and clinical outcomes.
• Track assessment schedules and submission deadlines.
• Help maximize reimbursement while maintaining resident-centered care.
• Serve as a resource for team members regarding assessment and care planning processes.

As our MDS Coordinator, you'll play a key role in supporting resident care, regulatory compliance, and reimbursement accuracy. Working closely with the interdisciplinary team, you'll help ensure assessments, care plans, and documentation reflect each resident's individual needs while promoting the highest standards of quality care.

What We’re Looking For

• Active Registered Nurse (RN) license required
• License must be current and in good standing
• Current CPR certification required

• Ability to manage multiple priorities and deadlines
• Knowledge of MDS, care planning, and long‑term care regulations preferred
• High personal integrity and professionalism
