Job summary

Doctors Hospital of Augusta, part of HCA Healthcare, seeks a Clinical Rehab Liaison (PRN) to coordinate referrals and patient placement for the inpatient acute rehab program. You will build relationships with case managers, physicians, and payors, ensuring timely assessments and smooth admissions.

Ideal candidates hold RN or equivalent licensure, a bachelor/associate degree, and 2 years in rehab settings, with preferred discharge planning experience.

Qualifications

• BLS certification required within 30 days of employment start date.
• RN or Allied Health professional licenses as listed.
• Bachelor's or Associate degree required.
• Minimum 2 years experience in rehabilitation or related settings.
• Prior acute rehab, subacute rehab, LTAC, or discharge planning experience preferred.
• Prior Clinical Rehab Liaison or acute care discharge planning experience required.

Responsibilities

• Coordinate between internal referrals and acute inpatient rehab operations.
• Establish relationships with case managers, physicians, payors, and other referrals.
• Work with Case Management and Rehab Medical Director for patient placement and screenings.
• Ensure smooth referral and admission flow for the inpatient rehab program.

Job Summary and Qualifications

This position acts as a coordinator between internal and external referral sources, and acute inpatient rehab operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors, and other referral sources. The Clinical Rehabilitation Specialist will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director.

The Clinical Rehabilitation Specialist works closely with hospital case management to ensure a responsive and smooth flowing referral and admission process for the inpatient acute rehab program.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• (RN) Registered Nurse, or (RRT/RCP-State) Respiratory Therapist/Practitioner, or (NBRC-RRT) Registered Respiratory Therapist, or (OT) Occupational Therapist, or (LPT) Licensed Physical Therapist, or (SLP) Speech-Language Pathologist
• Bachelors Degree, or Associate Degree
• 2 years experience required
• Prior acute rehab, subacute rehab or LTAC experience preferred
• Prior Clinical Rehab Liaison,or acute care hospital discharge planning experience required
