Job Summary

The Clinical Registered Nurse supports the goals of the care enhancement program by providing high-quality telephonic support in an efficient and cost-effective manner, ensuring high-quality care. The nurse coordinates with Primary Care Providers and health care team members to support appropriate healthcare management, resource utilization, and quality outcomes. Responsibilities include early identification of at-risk participants and intervening with participants and their providers according to disease management guidelines.

Supervised by an Operations Manager, all Sharecare clinicians participate in orientation, competency reviews, and annual testing.

Available Shifts

• Monday-Friday: 7am-3:30pm CT and every 3rd Saturday from 8am-4:30pm CT.
• Monday-Friday: 11am-7:30pm CT and every 3rd Saturday from 8am-4:30pm CT.
• Monday-Friday: 11:30am-8pm CT and every 3rd Saturday from 8am-4:30pm CT.

Training

Four weeks from 8am – 4:30pm CT. Attendance is mandatory during the first 60 days. Hours are on Central Standard Time.

Qualifications

• Current RN license for the state of employment, with ability to obtain licensure in other states as required.
• High School diploma and Nursing diploma, Associate’s Degree, or Bachelor’s Degree in Nursing (BSN preferred).
• Minimum of 3 years recent clinical experience.
• Basic knowledge of physiology and chronic diseases.
• Effective communication skills, both oral and written.
• Basic computer skills and familiarity with Windows OS.
• Ability to educate and promote behavior change in a telephonic environment.

• Review participant health exams and initiate interventions.
• Contact participants for assessment and education.
• Develop and implement care plans addressing healthcare needs.
• Identify and address barriers to healthcare access.
• Coordinate with healthcare teams to ensure continuity of care.
• Participate in team communication and collaboration.
