Job summary

UnitedHealth Group's Home Health division is seeking a CORE Clinical Coordinator to support administrative and operational activities in the home health referral process. The role ensures referrals are complete, timely, and accurately transitioned to the agency for evaluation and care.

Responsibilities include reviewing referrals, providing clinical input, triaging calls, and using systems like Forcura, e-portals, and Homecare Homebase to keep teams informed and enable timely admissions.

Qualifications

• Clinical background required.
• Strong organizational and time-management skills.
• Excellent communication and telephone skills.
• Familiarity with referral systems (Forcura, e-portals, Homecare Homebase).
• Knowledge of clinical practices and admission criteria.

Responsibilities

• Receives and reviews referrals for timely and accurate responses.
• Provides clinical review and may assist with verbal orders as needed.
• Ensures referrals include all required elements.
• Identifies missing criteria and follows up with teams.
• Triages incoming calls and enters referrals into the operating system.
• Communicates accurate referral information within CORE and to teams.
• Acts as liaison between operations and business development.
• Supports admission criteria to facilitate timely decisions and admissions.
• Provides information about agency services to patients and families.
• Ensures non-admits are labeled timely and accurately.
