Qualifications

  • Associate's degree in nursing required; BSN preferred.
  • 2–3 years of experience in home health or hospice quality assurance or clinical documentation.
  • Must hold a current license to practice as RN, PT, or OT in Vermont.
  • Valid driver's license.

Responsibilities

  • Review patient charts, Oasis assessments, plans of care for accuracy.
  • Identify and resolve documentation discrepancies to prevent deficiencies.
  • Audit medical records and care processes; develop quality improvement initiatives.
  • Verify alignment between orders, Plans of Care (485), and OASIS documentation.
  • Ensure compliance with Medicare, Medicaid, CLIA, OSHA, HIPAA, and Vermont home care rules.
  • Support the agency's QAPI program with data collection and trending.
  • Collaborate with nurses, therapists, social workers; participate in QA meetings.

Who we're seeking:

Join our Quality team as a Clinical Quality Lead, where you'll play a key role in safeguarding clinical documentation integrity, supporting regulatory compliance and reporting processes, and leading quality assurance and performance improvement initiatives. This position is based at our Barre, VT office. This position offers the opportunity to work across clinical and administrative departments, support strategic quality decision-making, and contribute to the continuous improvement of patient care documentation and outcomes.

Your Responsibilities:

As a member of our dedicated team, you'll:

  • Review and verify patient charts, OASIS assessments, plans of care, and other clinical documentation for completeness, accuracy, timeliness, and adherence to best practices.
  • Identify and resolve documentation discrepancies before they lead to survey deficiencies, claim denials, or negative patient outcomes.
  • Conduct audits of medical records and care processes, identify trends, and develop quality improvement initiatives to educate clinical staff.
  • Verify alignment between physician orders, the Plan of Care (485), and OASIS/clinical documentation.
  • Ensure compliance with Medicare, Medicaid, CLIA, OSHA, HIPAA, and Vermont Home Care Rules.
  • Support the agency's QAPI program, including data collection, trending, and performance improvement planning.
  • Compile and analyze quality indicators and performance metrics to recommend corrective actions, including those tied to PDGM and HHVBP reimbursement accuracy.
  • Collaborate with nurses, therapists, and social workers to ensure documentation reflects actual patient care and participate in QA meetings and interdisciplinary committees.

Requirements:
  • Associate's degree in nursing required; BSN preferred (a licensed PT or OT with home health/hospice clinical and quality experience will also be considered).
