Job summary

GuideStar Eldercare seeks a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to ensure holistic, continuous care for elderly residents with neurocognitive disorders and other neurological or psychiatric conditions.

The role emphasizes working within an interdisciplinary treatment team, autonomous function with Area Manager

Qualifications

• Master’s degree in Mental Health Counseling or Social Work.
• Currently licensed LMHC or LISW in the state you are applying to.

Job Description:

As a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.

Required Qualifications:

• Ability to work as part of an interdisciplinary treatment team

• Good time management skills

• Able to function autonomously with regional Area Manager support

• Master’s degree in Mental Health Counseling or in Social Work

• Currently licensed as an LMHC or LISW in the state in which you are applying

Preferred Skills:

• Passion for geriatrics

• Experience in inpatient settings such as extended care facilities

• Experience with brief bedside assessment screens (cognitive, mood, etc.)

• Motivated by productivity-based positions

• Ability to navigate and document on an Electronic Medical Record (EMR) system
