Job summary
CommUnityCare Health Centers is seeking a Program Manager for the Permanent Supportive Housing Health Care Collaborative (PSH HCC) in Travis County. The role leads clinical care services, shaping program design, staff guidance and quality improvement to support individuals transitioning out of homelessness.
The position partners with leadership to ensure integrated care, data-driven performance and effective stakeholder collaboration across sites.
Qualifications
- Experience leading clinical projects and operations in healthcare.
- Experience with homeless or marginalized populations is required.
- Proven ability to coordinate care with internal and external partners.
- Strong data and performance reporting experience.
Responsibilities
- Analyze population health data to improve workflows and outcomes.
- Lead development of KPIs, deliverables and outcome reporting.
- Oversee program implementation, oversight and quality improvement.
- Coordinate activities with internal teams and partners.
- Prepare program reports and data summaries for leadership.
- Monitor budgets and expenditures for funding alignment.
- Apply trauma-informed, culturally responsive, and harm reduction principles.
- Represent the program in organizational and community settings.
- Guide onboarding, training, huddles and performance feedback.
