Role Overview

Seeking a Clinical PAC SME to support post-acute care strategy, implementation, and optimization within inpatient case management initiatives. This is a client-facing role focused on improving care delivery, utilization, and financial outcomes through hands-on advisory and program leadership.

Key Responsibilities

• Lead design, implementation, and optimization of Post-Acute Care programs
• Provide program and engagement management across multiple workstreams
• Partner with client stakeholders, clinicians, and senior leadership to drive outcomes
• Support initiatives to improve care efficiency, post-acute utilization, and cost performance

Required Qualifications
