In this role you will provide direct patient care, participate in teaching and committee activities, mentor clinical interns, and collaborate across departments to optimize treatment plans and outcomes.

Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois.
• BLS from American Heart Association upon hire.
• Masters or Doctoral Degree in Physical Therapy (preferred).

Responsibilities

• Demonstrates NM values in daily work and interactions.
• Provides direct patient care and participates in committees and process improvement.
• Serves as a role model for therapists and interns in professional, clinical, communication, education and leadership activities.
• Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA(s).
• Documents in EMR and completes daily notes reflecting patient response and status.
• Participates in education of department/clinic and supports student clinical programs.
• Works with healthcare teams to develop and maintain professional relationships and to improve services.

Education

Bachelor’s Degree in Physical Therapy

Licensed Physical Therapist in State of Illinois

Masters or Doctoral Degree in Physical Therapy
