Qualifications

• Bachelor’s, Master’s, or entry level Doctorate of Physical Therapy degree from an accredited college or university in the area of Physical Therapy is required.
• All modalities/equipment used by a Physical Therapist as outlined by the Executive Council of Physical Therapy and Occupational Therapy Examiners.

Responsibilities

• Performs comprehensive diagnostic evaluations to determine the status of the physical, cognitive abilities of inpatients and outpatients.
• Develops and implements individualized treatment plans with patient/family participation and measurable goals.
• Adheres to documentation procedures including Initial Evaluations, Daily/Weekly progress notes, Discharge Summaries.
• Participates in patient care conferences and departmental meetings.
• Counsels patient and family regarding evaluation, care, and treatment; completes discharge planning and education.
