Qualifications

• Bachelor’s, Master’s, or entry level Doctorate of Physical Therapy degree from an accredited college or university.
• All modalities/equipment used by a Physical Therapist as outlined by the Executive Council of Physical Therapy and Occupational Therapy Examiners.

Responsibilities

• Performs comprehensive diagnostic evaluations to determine status of physical and cognitive abilities.
• Develops and implements individualized treatment plans with patient/family participation and measurable goals.
• Adheres to documentation standards including initial evaluations, progress notes, and discharge summaries.
• Participates in patient care conferences and departmental meetings.
• Counsels patients and families regarding evaluation, care, and treatment; initiates discharge planning.
