Qualifications

Bachelor's degree in Physical Therapy, required.
Licensed Physical Therapist (PT) in IL, or able to obtain.
Basic Life Support (BLS) within 30 days of hire.
Completion of required continuing education for acute stroke care.

Responsibilities

Demonstrates ability to transfer patients with safe body mechanics.
Delivers patient-centered care with empathy and engagement.
Designs and adjusts care plans with clear discharge goals.
Educates patients and families using effective teaching methods.
Assesses patients to determine PT diagnoses and outcomes.
Performs examinations using reliable methods relevant to chief complaint/history.
Documents in accordance with CTSCP67 policy.
Screens patients to determine therapy needs and effectiveness.
Supervises PTA and rehab technicians per regulations.
Communicates status, recommendations, and equipment needs with medical team.
