Job summary

Qualifications

• Graduate of a CAPTE accredited Physical Therapy Program or equivalent.
• Excellent written and verbal communication skills.
• Proficient computer skills that may include Microsoft Office and/or Google Suite platforms.

Responsibilities

• Performs physical therapy examination, evaluation, intervention planning and implementation.
• Collaborates with interdisciplinary team, educates patients and families, and documents care.
• Establishes a plan of care with goals and prognosis based on evaluation results.
• Initiates discharge planning and conducts outcomes analysis.
• Maintains accurate patient care data following regulatory guidelines.
