Qualifications

• Current state licensure as a Physical Therapist.
• Bachelor, Master or Doctoral degree in physical therapy.
• One year physical therapy experience preferred.

Responsibilities

• Conducts screening of patients at regular intervals to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets data, formulates goals and treatment plans with patient and family.
• Provides physical therapy services per PT guidelines and clinic policy.
• Monitors patient response and modifies treatment to attain goals.
• Educates patient, family and staff; provides in-service education as needed.
• Documents results of assessment, treatment, follow-up and termination.
• Ensures infection control program compliance and safety procedures.
