Description

The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:

(1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team.

(2) Population Management - The Clinical Pharmacist Specialist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required qualifications:

  • Bachelor's Degree in PHARMACY from an accredited School of Pharmacy. Or

  • PharmD in PHARMACY from an accredited School of Pharmacy.

  • Upon hire: Washington Pharmacist License

  • Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of starting the role

  • Upon hire: Pharmacy preceptor license in applicable state as applicable

  • 2 years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR -

  • 5 years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY

  • 1 and PGY-2 residency requirement.

Preferred qualifications:

  • Coursework/Training and Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience Or equivalent educ/experience
