Qualifications

• Pharmacist licensure required and BPS certification preferred
• Experience in clinical pharmacy settings advantageous

Responsibilities

• Analyze reports and EMR data to identify opportunities to improve outcomes and reduce costs
• Provide MTM services and develop personalized health improvement plans
• Collaborate with providers to implement clinical quality data initiatives
• Maintain 5-STAR quality metrics and participate in committees and audits
• Educate providers and members on formulary changes and health plan strategies
