Job summary

Abrazo Central Hospital in Phoenix is seeking a Clinical Pharmacy Specialist to develop, implement, and evaluate clinical pharmacy services across the facility. You will lead education for professional staff, participate in patient care decisions, and mentor students with the Director of Pharmacy guiding overarching objectives.

Qualifications

• Pharmacy practice residency or equivalent clinical experience.
• Current Pharmacist license in the State of Arizona.
• BL S certification and preferred clinically oriented credentials.

Responsibilities

• Participates in critical care rounds, optimizing drug therapy.
• Develops and implements evidence-based best practices and order sets.
• Provides education to medical, nursing, and pharmacy staff on clinical initiatives.
• Supports Pharmacy and Therapeutics Committee meetings with presentations.
• Conducts medication use evaluations in areas of need.
• Precepts pharmacy students and mentors staff pharmacists.
