Job summary

Mercy Springfield is seeking a Clinical Pharmacy Specialist in Ambulatory Care to collaborate with physicians and care teams to optimize medication therapy and improve patient outcomes. The role emphasizes education, quality improvement, and leadership in ambulatory settings.

You will serve as a clinical resource, precept pharmacy students and residents, and contribute to scholarly activities while supporting Mercy's mission and service standards.

Qualifications

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited School of Pharmacy.
  • Current Pharmacist license in the state of practice.
  • Missouri pharmacists must obtain a Medication Therapy Services Certificate within 90 days of hire.
  • Minimum of 5 years of active clinical practice in a specialty area.
  • Ambulatory Care experience strongly preferred.
  • Active Board of Pharmacy Specialties (BPS) Certification preferred, ideally within specialty practice area.

Responsibilities

  • Review patient records and evaluate medication therapy appropriateness.
  • Develop and implement individualized medication treatment plans.
  • Identify, prevent, and resolve medication-related problems.
  • Monitor patient outcomes and adjust pharmacotherapy recommendations as needed.
  • Conduct comprehensive medication assessments and Medication Therapy Services.
  • Collaborate with physicians and healthcare providers on drug selection, dosing, administration, contraindications, and drug interactions.
  • Provide medication counseling and education to patients and caregivers.
  • Serve as a clinical resource and subject matter expert within ambulatory care practice areas.
  • Partner with physicians, nurses, and care teams to ensure optimal patient outcomes.

Requirements & Qualifications
Education
  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited School of Pharmacy
Licensure
  • Current Pharmacist license in the state of practice
  • Missouri pharmacists must obtain a Medication Therapy Services Certificate within 90 days of hire
Experience
  • Minimum of 5 years of active clinical practice in a specialty area
  • Ambulatory Care experience strongly preferred
Certifications
  • Active Board of Pharmacy Specialties (BPS) Certification preferred, ideally within specialty practice area
  • Candidates eligible to sit for the certification exam will also be considered
Preferred Qualifications
  • Five or more years of advanced clinical pharmacy experience
  • Completion of residency training preferred
  • Experience serving as a clinical preceptor for pharmacy students and residents
  • Recent scholarly activity, including:
    • Poster presentation at a state or national meeting within the last two years, and/or
    • Publication in a state or national journal
  • Teaching appointment with an affiliated healthcare college (pharmacy, nursing, medicine, or related field) preferred

What You'll Do
Clinical Pharmacy Practice
  • Review patient records and evaluate medication therapy appropriateness
  • Develop and implement individualized medication treatment plans
  • Identify, prevent, and resolve medication-related problems
  • Monitor patient outcomes and adjust pharmacotherapy recommendations as needed
  • Conduct comprehensive medication assessments and Medication Therapy Services
  • Collaborate with physicians and healthcare providers on drug selection, dosing, administration, contraindications, and drug interactions
  • Provide medication counseling and education to patients and caregivers
  • Serve as a clinical resource and subject matter expert within ambulatory care practice areas
Interdisciplinary Collaboration
  • Partner with physicians, nurses, and care teams to ensure optimal patient outcomes
  • Participate in ministry-wide clinical initiatives, specialty councils, Pharmacy & Therapeutics committees, and quality improvement efforts
  • Maintain strong working relationships across departments and care settings
Education & Leadership
  • Serve as an active preceptor for pharmacy students and residents
  • Support continuing education through presentations, lectures, and staff training
  • Assist in developing staff competencies and clinical education programs
  • Contribute to scholarly and professional development activities
Quality, Safety & Compliance
  • Monitor medication use and intervene when clinically appropriate
  • Participate in medication use evaluations, adverse drug event reporting, and quality assurance initiatives
  • Support regulatory compliance and accreditation standards
  • Adhere to confidentiality, patient safety, and risk management policies
Technology & Documentation
  • Utilize Epic and pharmacy information systems for clinical documentation and order verification
  • Leverage clinical resources such as Micromedex and other drug information databases
  • Maintain accurate patient records and therapy documentation
