Job Description
The Specialized Clinical Pharmacist, under the direction of the Director of Pharmacy Services, manages the clinical pharmacy section of the division. This includes developing, monitoring, evaluating, and coordinating clinical pharmacy services to promote desirable patient outcomes within the Indus Health Network.
Essential Responsibilities as a Clinical Pharmacist:
- Establish, coordinate, implement, and provide all aspects of clinical pharmacy activities, emphasizing therapeutic drug monitoring, clinical pathways, formulary management, pharmacoeconomics, and patient outcomes through detailed reporting systems.
- Assist the Director in developing operational systems and procedures for the Pharmacy Department’s Clinical Division, in accordance with guidelines from the Joint Commission on Accreditation of Healthcare Organizations and state hospital codes.
- Coordinate activities related to the provision of safe, effective, and economical pharmacotherapy for patients on anti-infective agents and other monitored drugs.
- Collaborate with physicians to establish drug criteria for safe, economical, and appropriate medication use.
- Participate in multidisciplinary team rounds and provide pharmacotherapy recommendations during discussions.
- Develop disease and drug therapy management programs to ensure safe, appropriate, and cost-effective medication use.
- Manage non-formulary drug requests and participate in Pharmacy & Therapeutics Committee activities.
- Perform initial assessments of drugs with pre-established monitoring criteria and conduct follow-up monitoring.
- Participate in pharmacoeconomic evaluations and outcomes research.
- Retrieve, analyze, evaluate, and interpret scientific literature; serve as a resource for patient and population-specific drug information to healthcare professionals and patients.
- Assist the Director in coordinating reviews, tracking, and compiling data on adverse drug events, including medication incidents and reactions.
- Issue statements on clinical policies and procedures of the Pharmacy Department.
- Assist in the selection of drug products and development of protocols.
- Participate in generating new knowledge related to pharmacotherapy, clinical pharmacy, and medicine; educate healthcare professionals, students, patients, and the public on rational drug therapy.
- Interpret and apply pharmacokinetic data, recommend dosing regimens based on patient history and disease state to ensure optimal care.
- Develop communication systems to respond to problems or emergency requests from patient care units.
