Position Overview

As part of a dedicated multidisciplinary team, the cardiology transitional care clinical pharmacy manager provides inpatient and outpatient clinical services, collaborating to achieve program outcomes such as reduced readmissions and emergency department utilization in patients with heart failure and acute myocardial infarction.

Responsibilities

1. Assess healthcare needs and resolve medication-related problems.
2. Collaborate with inpatient and outpatient teams for effective care coordination.
3. Manage post-discharge transitional care visits via telemedicine or remote patient monitoring for a cohort of heart failure and AMI patients.
4. Optimize evidence-based pharmacotherapy through collaborative drug therapy management (CDTM) agreement.
5. Serve as a resource for transitional care information and follow-up coordination for patients and caregivers.
6. Act as a liaison to post-acute clinical partners, including home care and skilled nursing facilities.
7. Engage in performance and quality improvement activities, promoting excellence in transitional care.

Qualifications

1. Doctor of Pharmacy degree.
2. Advanced training, such as PGY-2 residency in cardiology, ambulatory care, or relevant specialties, or PGY-1 residency with current board certification and 3 years of relevant experience.
3. NYS license with eligibility for credentialing as an allied health professional.
