Job summary

WVUH West Virginia University Hospitals seeks an experienced pharmacist to provide comprehensive pharmaceutical care across inpatient settings. The role involves direct patient care, education, and leadership in pharmacy operations to support safe, effective medication use.

Responsibilities include monitoring drug therapy, participating in rounds, guiding other clinicians, and ensuring compliance with safety procedures.

Qualifications

• PharmD from ACPE accredited school preferred.
• Must have current pharmacist licensure in WV, PA, MD or OH.
• BLS certification within 60 days of hire.
• Experience in hospital/pharmacy leadership roles preferred.

Responsibilities

• Monitors drug therapy and participates in rounds to evaluate regimens.
• Provides leadership and guides care to ensure safe, effective drug use.
• Assists with admission/discharge planning and patient education on medications.
• Delivers drug information and educational programs to staff.
• Maintains professional competence through CE, literature review, and research dissemination.
• Participates in QA, PI, and clinical research activities within the department.
• Reviews orders for accuracy and appropriateness of medications.
• Ensures compliance with medication safety procedures and policies.
• Documents errors and near misses to promote safety culture.
• Promotes a medication safety-focused environment.
• Dispenses medications per provider orders and follows distribution policies.
