Job description

Remote with some travel to Greater Charlotte Market acute care facilities

FT Virtual Pharmacy Services for Southeast Region. Atrium Health Pharmacists will need to be licensed by Georgia or North Carolina to start. Must be able to work a Rotating/Variable schedule which includes BOTH 1st AND 2nd shift rotation. Rotating Weekends and Holidays are required.

Virtual Pharmacists will verify orders for all acute care facilities in the Southeast Region, including rehabilitation hospitals. There will be Med Surg, Critical Care, and Pediatric order verification queues built in Epic.

Oversees the provision of safe and effective medication therapies through collaboration with the patient’s care team. Reviews physician orders for drug therapy and monitors this therapy for drug choice, dosage, drug interactions, adverse drug reactions, and formulary adherence. Serves as a drug information resource to nurses, physicians, and other healthcare workers.

Essential Functions

  1. Retrieves and evaluates relevant biomedical literature, formulates an appropriate response, and provides concise and timely response to drug information requests.
  2. Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration.
  3. Provides accurate shift-to-shift communication regarding unresolved situations requiring future action.
  4. Collects information needed to design medication therapy regimens and determine the presence of medication-related problems with current therapies.
  5. Designs, recommends, monitors, and evaluates patient-specific pharmacotherapeutic regimens. Implements recommendations effectively and utilizes existing clinical intervention policies and procedures, documenting in the medical record using the appropriate format.
  6. Participates in the management of medical emergencies.
  7. Precepts pharmacy students and pharmacy residents.
  8. Prepares and dispenses medications in accordance with best practices, policies, and state and federal regulations. Manages medication therapies using clinical guidelines.
  9. Checks the accuracy of the work of pharmacy support team members or others in accordance with the organization’s policies and procedures.

Physical Requirements

Works primarily in the pharmacy. Delivers medications to nursing units. Requires considerable walking, standing, pushing mobile carts up to 50 pounds, and lifting 25 pounds of material. May need to stretch, bend, squat, or kneel frequently. Uses PPE to minimize exposure to hazardous agents. Takes precautions against needle sticks, cuts, or bruises. Exposed to conditions like walking on hard surfaces, climbing stairs, and temperature/humidity changes. Must perform manipulative skills such as writing, typing, and data entry. Must have use of both hands, clear vision for reading small print, and effective communication skills. Must be able to hear normal sounds with moderate background noise. Involves rotating shifts.

Education, Experience, and Certifications

Bachelor's degree in pharmacy or Pharm D. from an accredited school required. Pharmacist license issued by the practicing state required. Hospital pharmacy experience preferred. Strong communication and interpersonal skills required. Two years of acute care hospital pharmacy experience, PGY-2 residency, or equivalent experience preferred. Willingness to train and join a Virtual Pharmacy Services team preferred.
