Job summary

U.S. Department of Veterans Affairs in Fort Meade and Hot Springs is seeking a Clinical Pharmacist to perform medication preparation, dispensing, and counseling.

The role includes completing medication histories, documenting in the EHR, and extending refills under the provider’s name, with responsibilities in emergency response and therapeutic drug monitoring. The position requires ACPE-accredited pharmacy education (BS Pharmacy or PharmD), full currentlicensure, and English proficiency.

Qualifications

• Graduate of ACPE-accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
• Licensure: Full - current and unrestricted license to practice pharmacy in a State or D.C.
• English Language Proficiency: Pharmacists must be proficient in spoken and written English
• GS-12 level requires 1 year of experience at GS-11 or 2 years at GS-12-equivalent or completion of ACPE-accredited Pharm.D.

Responsibilities

• Performs medication preparation and dispensing and provides medication counseling
• Completes medication histories including patient interviews
• Documents activities in the EHR and extends refills under the provider's name
• Maintains pharmacotherapy with monitoring and evaluation of drug therapy
• Participates in emergencies and clinical consultations with healthcare professionals
