Position Summary

Clinical Pharmacist - Transitions of Care

We're seeking a Clinical Pharmacist - Transitions of Care to provide safe, effective pharmaceutical care while collaborating with healthcare professionals to support our patients.

What You'll Do

• Dispense medications and IV fluids according to established standards and procedures


• Develop and monitor pharmaceutical care plans


• Provide pharmacotherapy and pharmacokinetic consultations, as applicable


• Utilise pharmacy and healthcare computer systems to support patient care


• Evaluate medication-related alerts, including IV-to-PO and renal dosing protocols


• Identify, investigate, and document adverse drug reactions and medication errors


• Participate in multidisciplinary rounds, when applicable


• Provide excellent service and communication to patients, families, physicians, visitors, and team members

Education

• Bachelor of Science in Pharmacy or PharmD required

Licensure

• Current Florida Board of Pharmacy license required

Experience

• Hospital pharmacy experience preferred
