Job Overview

Clinical pharmacists work closely with other health practitioners to meet patient needs by reviewing and approving medication orders, monitoring drug therapies, and providing drug information. They improve patient health by offering pharmaceutical care and serve as resources to physicians, nurses, and other health professionals. They also provide leadership in developing and maintaining hospital and departmental systems that promote desirable patient outcomes, educate students and support personnel, and participate in drug therapy research.

Job Description
Minimum Qualifications:
  1. Bachelor's degree in Pharmacy.
  2. Pharmacist License.
  3. One (1) year of related experience.
Preferred Qualifications:
  1. Doctor of Pharmacy (PharmD).
  2. Board Certification.
  3. Four (4) years of related clinical pharmacy experience with direct patient care, obtained through residency or prior employment.
  4. Experience in drug utilization review and analysis.
Duties and Responsibilities
  1. Directs and coordinates clinical activities within the pharmacy service area, including preparing, compounding, and dispensing medications per regulations.
  2. Reviews physician's orders for appropriateness, checking for indication, clarity, dose accuracy, allergies, interactions, and duplications.
  3. Communicates with physicians regarding medication orders and interventions, with outcomes documented routinely.
  4. Ensures medication preparation and labeling are accurate and supervises support staff during shifts, complying with regulations.
  5. Provides drug information utilizing pharmacology knowledge and scientific literature.
  6. Conducts medication history interviews, reconciles therapies, and educates patients and families.
  7. Anticipates controlled drug needs based on records.
  8. Completes medication station inspections and participates in medication education and drug therapy research.
  9. Performs order entry and utilizes pharmacy computer systems accurately.
  10. Participates in multidisciplinary team meetings, medication reconciliation, and provides optimization consultations.
  11. May conduct home visits for medication reviews and education.
  12. Participates in pharmacy quality programs related to various operational and clinical excellence areas.
  13. Provides operational support, including coverage during weekends, holidays, evenings, nights, and on-call.
  14. Provides continuing education, serves as preceptor, and participates in developing treatment guidelines and departmental initiatives.
Physical Requirements
  1. Frequent standing, walking, and lifting of 5-10 lbs.
  2. Lifting and carrying light loads, including boxes and equipment, and stooping/kneeling.
  3. Manual dexterity for operating medications and computers.
  4. Ability to see medications, screens, and reports clearly.
Skills & Abilities
  1. Knowledge of pharmacy laws, regulations, and standards, including inventory management.
  2. Excellent verbal and written communication skills, including presentations.
  3. Knowledge of health networks, drug formularies, and project management.
  4. Creative problem-solving skills and understanding of confidentiality standards.
  5. Proficiency in clinical best practices, Microsoft Office, and electronic medical records.
  6. Ability to work with diverse populations, manage chronic conditions, and utilize motivational interviewing.
  7. Strong organizational skills, attention to detail, and ability to prioritize issues.
  8. Willingness to learn new technologies and work independently or in teams.
