UT Health Tyler is seeking a daytime, full-time Clinical Pharmacist Specialist in Tyler, TX. The role focuses on safe, effective medication therapy and patient-centered care within a decentralized hospital setting. You will collaborate with physicians, nurses, and staff to optimize medication use.

The ideal candidate holds a Pharm.D. or ACPE-accredited degree, hospital clinical experience, and PGY-1/PGY-2 residencies (or equivalent 5+ years experience).

Qualifikationen

• B.S. or Pharm D. from ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of an accredited PGY-1 Pharmacy Residency or fellowship.
• Completion of an accredited PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field).
• OR Completion of an accredited PGY-1 Pharmacy residency or fellowship AND 3+ years of clinical pharmacy experience (if no PGY-2).
• OR 5+ years of clinical pharmacy experience with no residency/fellowship.
• Board of Pharmacy Specialties (BPS) certification (or within 1 year).
• Active Pharmacist license in good standing with State Board of Pharmacy.
• Other applicable state requirements such as sterile compounding.
• BLS/ACLS certification must be obtained within 14 days of hire.

Aufgaben

• Provide safe, effective, and prompt medication therapy.
• Deliver patient-centered care and integrated pharmacy services in a decentralized practice with physicians, nurses, and other hospital staff.
• Participate in all medication-use-system aspects while providing comprehensive pharmaceutical care.
• Support centralized and decentralized medication-use systems.
