Minimum Education and Experience Requirements:

Education:

• Doctor of Pharmacy
• Completion of PGY2 Pharmacy Specialty Residency PLUS 2 years of relevant experience; OR Completion of PGY1 Pharmacy Residency PLUS 4 years of relevant experience; OR No residency with 7 years of relevant experience

Experience:

• Pharmacist License in Pennsylvania
• Board Certification from the Board of Pharmacy Specialties preferred. Alternatively, specialized certification in clinical practice may be acceptable, to be evaluated on a case-by-case basis by the pharmacy supervisor.
