Job summary

University of Michigan Health in Ann Arbor seeks a Clinical Pharmacist Specialist to support ambulatory solid tumors. You will collaborate with Hematology/Oncology teams, educate patients, and participate in research and teaching activities.

You will provide evidence-based pharmacotherapy, engage in tumor board discussions, and help advance outpatient solid tumor care across the department.

Qualifications

Doctor of Pharmacy degree and license to practice in Michigan (or eligibility).
Completion from oncology residency/fellowship or equivalent training.
Clinical pharmacy experience required; oncology board certification desired.

Responsibilities

Provide care to ambulatory solid tumor patients (breast cancer, melanoma) within a multidisciplinary team.
Incorporate new treatments using evidence-based, standardized approaches; participate in tumor board discussions.
Conduct independent patient visits to educate on complex regimens and supportive care.
Operate under a Collaborative Practice Agreement for GI symptoms and participate in PRO text programs.
Coordinate outpatient delivery of solid tumor treatments; collaborate with inpatient teams.

Job Summary

As a Clinical Pharmacist, you will be part of the Department of Pharmacy Hematology/Oncology team, as well as multidisciplinary teams within the University of Michigan Rogel Cancer Center. University of Michigan Health is ranked nationally for the care provided to both adult and pediatric cancer patients, as well as many other specialties. The cancer center mission is the conquest of cancer through innovation and collaboration. Current hematology/oncology clinical pharmacists provide exceptional pharmaceutical care on the individual and population level. Responsibilities*

This pharmacist will:

Provide care to patients seen in the ambulatory solid tumor clinics with a focus on breast cancer and melanoma. As an integral member of the multidisciplinary team, collaborate with providers on crafting and personalization of treatment plans for patients with breast cancer, melanoma, and other solid tumor malignancies.
Guide incorporation of new treatments in a standardized evidence-based fashion, in conjunction with other members of the solid tumor pharmacist team, including involvement at tumor board discussions.
Provide independent visits with patients to educate on complex treatment regimens and provide supportive care.
Practice under a Collaborative Practice Agreement for nausea, vomiting, constipation, and diarrhea, as well as participate in a nausea and vomiting patient-reported outcome text messaging program.
Coordinate care to promote outpatient delivery of treatments for solid tumors.
Collaborate closely with inpatient solid tumor teams to provide patient centered care.

What skills would you master?

You will assure safe and evidence-based, cost-effective use of medications, as well as contribute to formulary review and maintenance
You will assist in the development of staff educational and training programs within the Pharmacy Department
As a Clinical Pharmacist, you will also be appointed as an adjunct faculty member at the University of Michigan College of Pharmacy where you will participate in the didactic and experimental training of pharmacy students and residents
You will work individually and/or collaboratively on scholarly/research activity

Required Qualifications*

A Doctor of Pharmacy degree or an equivalent combination of education and experience.
Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable in LARA; or eligibility to become licensed within 90 days of employment start date.
A certificate of completion from an oncology residency, fellowship, or equivalent combination of education, training, and experience is necessary.
Experience in clinical pharmacy practice is necessary.
Board Certification in Oncology Pharmacy is desired, but not required
