Job summary

Michigan Medicine is seeking a Clinical Pharmacist Infusion to join our ambulatory infusion services. You will perform medication order reviews, dispense and check sterile preparations, and provide in-services for nursing education.

Your expertise in oncology and non-oncology therapies, along with a PharmD or equivalent and state licensure, will support patient care across multiple infusion centers. This full-time role offers hybrid work options and a strong team environment, with weekend

Qualifications

• Doctor of Pharmacy Degree from an accredited College of Pharmacy or Bachelor's Degree in Pharmacy with commensurate experience.
• Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable in LARA; or eligibility to become licensed within 90 days of employment start date.
• Ability to perform a variety of manual dexterity movements, including keyboarding and IV preparation, actively participating in codes.
• Fluency in reading, writing and speaking English.
• Completion of a PGY1 residency and/or BCPS certification is desired
• Recent inpatient clinical hospital practice experience or home infusion pharmacy experience including IV admixture, direct patient care and education, with a range of medications and patient populations similar to UMHHC.

Responsibilities

• Participate in direct patient care with the healthcare team to assure appropriateness of therapy.
• Review medication profiles and provide patient counseling.
• Serve as preceptor for Pharm.D. students on rotations.
• Order verification/profile review and ensure timely processing of orders.
• Provide input on process improvements and lead implementation of changes.

Unit Based Responsibilities:

• You'll participate in direct patient care which includes working with members of the healthcare team to assure appropriateness of therapy. Other activities include reviewing medication profiles, patient counseling, and physician/nurse education.

Teaching:

• You will serve as a preceptor for Pharm.D. Students on a variety of rotations including medication reconciliation and direct patient care Introductory Pharmacy Patient Experiences (IPPEs) during the third year and institutional and unit-based hospital practice clerkship rotations during the fourth year.
• Order Verification/ Profile Review: You will review, clarify and process medication orders in a timely manner while assuring appropriateness of therapy.
• Quality Improvement: You will provide input and suggestions for process improvements to improve the care that is provided to our patients. Activities include researching opportunities and initiating action plans with follow through until successful implementation of the improvement.
• Supervision of Work Area: You will provide functional supervision of technical staff working in their area including completion of job assignments and assuring adherence to institutional policies and procedures.
• Patient Populations: You will service patients seeking oncology and non-oncology care. This would include Rheumatoid Arthritis, Multiple Sclerosis, Hematology, ALS, Lou Gehrig's Disease, Pulmonology and IBD.

Modes of Work

Work Schedule

This is a full-time position, rotating schedule. Rotation among work locations may occur. Michigan Medicine has ambulatory infusion services located at Brighton Center for Specialty Care, Children's and Women's Infusion Center, Rogel Cancer Center, East Ann Arbor Health Center, Northville Health Center, Taubman Center, and West Ann Arbor Health Center. Weekends and holidays will be required on a rotational basis. Remote work opportunities exist within the infusion pharmacy team and are also available on a rotational basis.

Background Screening
