CLINICAL PHARMACIST SPECIALIST, AMBULATORY CARE PULMONARY (FT; 40hrs/wk) - Temple University Hospital - ( 244919 )

The Clinical Pharmacist Specialist for Ambulatory Care is responsible for the provision of advanced Clinical Pharmacy services within an ambulatory care setting. Reporting to the Director of Pharmacy Clinical Services, Education & Residency, the Clinical Pharmacy Specialist will be responsible for the advanced clinical pharmacy services including: Facilitates and documents medication therapy management via recommendations and/or collaborative practice agreements. Manages drug therapy and utilization through transitions in care and monitors medication therapy outcomes. Contributes to specialty practice through committee participation and development of policies, procedures, and pharmacy practice protocols. Provides patient education and in-service education to health care professionals. Mentors pharmacy students, pharmacy practice residents, and pharmacy staff in areas related to acute care. Conducts pharmaceutical research in outpatient environments.

Education
Doctorate Degree : PharmD (Required)
Other : PGY1 Pharmacy Residency or a relevant combination of education and experience (Required)
PGY2 Pharmacy Residency in Ambulatory Care or Transitions of Care (Preferred)

Experience
2 Year experience in institutional practice or residency completion (Required)

License/Certifications
RPH-LIC - PA Pharmacist License (Required)
BCACP - Bd Cert Ambulatory Care Pharm (Preferred) or BCPS - Cert Pharmacotherapy Spec (Preferred)
ACLS - Advanced Cardiac Life Support (Preferred)
