Job summary

Dignity Health - Northridge Hospital Medical Center in Los Angeles is seeking a Clinical Pharmacist to deliver comprehensive pharmaceutical care, collaborate with physicians, nurses, and other professionals, and optimize medication therapy to improve patient outcomes.

You will review orders for safety and efficacy, monitor drug levels, provide patient education, and participate in antimicrobial stewardship and MTM services for patients with chronic diseases.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Residency completion or 3 years of clinical experience.
• Current CA Pharmacist Licensure required.

Responsibilities

• Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals.
• Educate patients on their medications, including proper administration, potential side effects, and drug interactions.
• Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases.
• Participate in antimicrobial stewardship programs to promote appropriate use of antibiotics and prevent antimicrobial resistance.
• Monitor for and report adverse drug reactions.

Job Summary and Responsibilities

As our Clinical Pharmacist, you will provide comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes.

Every day, you will be responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives.

To be successful in this role, you will review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies; monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity.

Job Requirements

Requirements:

• - Fire Safety card within 90 days of hire.
