Dignity Health - Northridge Hospital Medical Center is seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care to patients, collaborating with physicians, nurses and other professionals to optimize therapy and outcomes.

Responsibilities include medication order review, pharmacokinetic monitoring, patient education, MTM for chronic diseases, and antimicrobial stewardship participation. CA licensure and hospital experience are preferred to ensure safe, evidence-based care.

Qualifications

  • Completion of a 4-year course at an accredited School of Pharmacy with a BS or Pharm. D. degree.
  • At least two years of hospital pharmacy experience preferred or has completed an ASHP-accredited residency.
  • Current CA Pharmacist Licensure required.
  • Fire Safety card within 90 days of hire.

  • Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals.
  • Educate patients on their medications, including proper administration, potential side effects, and drug interactions.
  • Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases.
  • Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance.
  • Monitor for and report adverse drug reactions.

As our Clinical Pharmacist, you will provide comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes.


As our Clinical Pharmacist, you will provide comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes.
Every day, you will be responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives.
To be successful in this role, you will review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies; monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity.
