SCAN is seeking a clinical pharmacist to join a multidisciplinary team advancing care for seniors. The role supports high-risk members, partnering with physicians, nurses, and care teams to optimize medications and outcomes.
Expect remote/field work about 25% with opportunities to influence care quality and access. The candidate will review medications, deprescribe when appropriate, educate staff, and lead initiatives while maintaining compliance with regulations.

The clinical pharmacist is an essential member of the multidisciplinary care team, providing expertise to promote safe, evidence-based, and cost-effective drug therapy to our members to help advance Scan's goal of keeping seniors health and independent. In this role, you will be the pharmacist supporting a care team responsible for caring for some of Scan’s highest-risk and most complex members, partnering closely with physicians, advanced practice providers, nurses, social workers, and other care team members in order to dramatically improve patients’ health outcomes. The pharmacist also serves as a trusted clinical leader across the enterprise, helping drive quality, affordability, and positive member outcomes.
