Job summary

w3r Consulting seeks an experienced Clinical Pharmacist to support Medicare Part D Stars performance, medication adherence, and quality initiatives for physician practices in the Rio Grande Valley, Texas. This clinical role requires collaboration with physicians and clinical stakeholders without dispensing responsibilities.

The successful candidate will travel periodically, educate providers on value-based care, and contribute to residency preceptorship programs while improving patient outcomes

Qualifications

• Bachelor’s degree in Pharmacy required; PharmD strongly preferred.
• Active Texas pharmacist license in good standing.
• Minimum of 3 years of managed care and/or ambulatory care pharmacy experience.
• Experience serving patient populations within the Texas market.
• Strong verbal and written communication skills, including the ability to educate and influence clinical stakeholders.
• Ability to work independently, solve problems, prioritize responsibilities, and make timely decisions.
• Proficiency in Microsoft Word, Excel, and PowerPoint.
• Ability to work without employer-sponsored immigration support.

Responsibilities

• Support Medicare Part D Stars performance and medication utilization improvement initiatives.
• Conduct comprehensive medication reviews for patients with multiple chronic conditions.
• Identify drug therapy problems, medication safety concerns, and opportunities to improve medication adherence.
• Collaborate with physicians, advanced practice providers, and clinical teams to recommend evidence-based prescribing practices.
• Educate provider offices on value-based care initiatives, medication adherence strategies, and quality improvement opportunities.
• Analyze, interpret, and communicate medication utilization and performance data.
• Support clinical quality improvement initiatives designed to improve patient outcomes and reduce unnecessary healthcare costs.
• Build and maintain strong relationships with physician practices and clinical stakeholders.
• Contribute to the Managed Care Pharmacy PGY-1 residency program and provide clinical preceptorship for pharmacy residents and/or students, as applicable.
• Conduct regular provider office visits throughout the assigned market and travel to Laredo, TX, approximately once per month.
