AdventHealth in Tavares, Florida, seeks a licensed pharmacist to educate patients and healthcare teams on safe, effective medication use and to participate in direct patient care during a PRN Day shift. The role includes supervising staff, contributing to safety initiatives, and ensuring smooth transitions of care.

The position requires a PharmD (Bachelor’s with license acceptable) and hospital experience; benefits begin Day One and support ongoing professional development.

Formación

  • PharmD degree required; Doctorate preferred.
  • Licensed Pharmacist (RPH) required.
  • 1+ hospital experience strongly preferred.

Responsabilidades

  • Educate and counsel patients, families, and healthcare team on safe medication use.
  • Provide safe, timely, and cost-effective medication therapy; participate in stewardship and surveillance.
  • Engage in direct patient care and interdisciplinary rounds; ensure smooth transitions of care.
  • Supervise and mentor pharmacy staff; support team development.
  • Identify and help resolve medication safety events and errors.
  • Demonstrate proficiency with pharmacy informatics systems and medication automation.
  • Contribute to practice advancement through research and scholarly activity.
  • Perform pharmacist functions including compounding, dispensing, verification, and documentation.

Job Description
  • Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
  • Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs.
  • Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow‑up.
  • Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
  • Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
  • Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation.
  • Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity.
  • Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
  • Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug‑related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
  • Other duties as assigned.
  • Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
