The role you’ll contribute:
Prepares and organizes pharmacy medications by name and dosage, as listed on patient orders.

Pharmacy Operations
Pharmacy Practice
Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems
Proficiently verifies/enters medication orders and proactively communicates with medical staff to resolve problem medication orders
Provides patient ‐ centered care services
Initiates and facilitates appropriate therapy based on age, disease, and drug considerations
Promotes cost-effective drug therapy and actively participates in stewardship activities and restricted medication/surveillance programs
Facilitates medication safety practices
Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors
Documents pharmacist interventions and appropriately documents in the patient medical record
Clinical Pharmacy Services
Provides support to direct patient care programs
Provides accurate, safe, timely, and appropriate medication therapy based on patient need
Places practice priority on the delivery of patient-centered care
Supports practice in decentralized areas
Collaborates with other pharmacy staff
Collaborates with care providers and Pharmacists to optimize care and improve services
Qualifications
The expertise and experiences you’ll need to succeed :
· Bachelors and 1+ years experience
· Texas Licensed Pharmacist
