Qualifications

• Doctorate in Pharmacy (PharmD) or BS for those licensed under BS license requirement.

Responsibilities

• Admin: prior authorizations, appeals, patient assistance program enrollment, billing, drug information.
• Clinical: Medication management, medication reconciliation, disease state management, patient counseling, drug therapy consultation.
