Qualifications

• Pharm.D. or B.S. Pharmacy with equivalent experience.
• Current licensure in the State of Pennsylvania or eligible for licensure.
• ASHP-Accredited Pharmacy Practice Residency or equivalent preferred.
• Equivalent of three years' hospital pharmacist experience.

Responsabilités

• Evaluate drug therapy with physicians and allied health staff to improve outcomes and contain costs.
• Identify drug therapy strategies to improve patient outcomes and reduce costs.
• Monitor responses to drug therapy in medically complex elderly patients.
