Qualifications

• Licensed pharmacist in state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required.
• Credentialing and privileging required based on area of practice.
• Bachelor’s or Doctorate in Pharmacy from ACPE-accredited school or NABP FPGEC certificate.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on medications, adherence, and safety.
• Interprets medication orders accurately and participates in emergencies as needed.
