Qualifications

• Requires Doctorate of Pharmacy or Bachelor of Science in Pharmacy from an accredited college.
• Must hold a current valid license to practice pharmacy in Pennsylvania.
• Continuing education credits to maintain active licensure as mandated by the PSBP.

Responsibilities

• Review and verify physician medication orders for accuracy, appropriateness, and interactions.
• Prepare and dispense medications, including IV admixtures, chemotherapy, and controlled substances.
• Monitor patient medication therapies and provide clinical consultations to medical and nursing staff.
• Participate in rounds and collaborate with care teams to optimize medication use.
• Educate patients and families on safe medication use when appropriate.
• Supervise pharmacy technicians, students, and interns.
• Ensure compliance with state, federal, and institutional regulations.
• Maintain accurate documentation in pharmacy and patient records systems.
• Participate in quality improvement and safety initiatives within the pharmacy department.
• Participate in CDTM programs as assigned.
