The Clinical Excellence Quality (CEQ) Pharmacy Operations team supports Optum’s Care Delivery Organizations by providing patient-centered, clinically collaborative medication consultations to promote healthier lives. Our team focuses on telephonic and digital engagement strategies to close Medicare star measure care gaps. The team includes Pharmacists, RNs, Pharmacy Technicians, and Care Coordinators.

Clinical pharmacists perform telephonic, patient-centered consultations focusing on medication adherence, SUPD/SPC, care of older adults, A1c reviews, and polypharmacy measures, including opioids/benzodiazepines and anticholinergic medications. They also consult with providers, pharmacies, and other stakeholders to improve patient outcomes and address social determinants of health.

Requirements include meeting productivity and quality metrics, with flexible schedules supporting hours from Monday to Saturday, 9:00 AM – 6:00 PM CT, including rotating Saturday shifts (~6 per year).

Primary Responsibilities
  1. Conduct telephonic medication consultations exceeding production and quality standards.
  2. Perform annual medication reviews emphasizing patient assessment, medication safety, and drug interactions.
  3. Assess pharmacotherapy based on claims, medical history, and disease states.
  4. Identify adherence barriers and develop plans to improve medication success.
  5. Provide evidence-based therapeutic recommendations to providers.
  6. Deliver patient-centered, collaborative medication consultations.
  7. Create post-consultation action plans.
  8. Document consultations in internal platforms.
  9. Participate in staff meetings and development activities.
  10. Suggest system improvements for operational efficiency.
  11. Maintain current knowledge on chronic diseases and pharmacy practices.
  12. Assess customer needs and develop solutions.
  13. Handle non-standard requests independently.
  14. Provide explanations and mentorship to team members.
  15. Perform other duties as assigned.

Required Qualifications
  1. Active, unrestricted pharmacist license in your primary state; willingness to obtain additional licenses.
  2. 5+ years pharmacy practice, including 2+ years clinical experience.
  3. Experience with Medicare STARs & HEDIS measures, MTM, or COA reviews.
  4. Proficiency in Microsoft Office Suite.
  5. Ability to manage multiple projects and adapt to change.
Preferred Qualifications
  1. Doctor of Pharmacy degree.
  2. Multi-state licensure (ID, OR, TN, KY).
  3. Post-graduate training or residency.
  4. Board certification (BPS).
  5. Call center experience.
  6. Bilingual in Spanish.
  7. Strong communication and organizational skills.
  8. Ability to work independently and collaboratively.

The salary range is $89,800 to $176,700 annually, based on various factors. Benefits include comprehensive packages, incentives, stock options, and 401k. The application deadline is at least 2 business days from posting, subject to early removal if filled.
