Job summary

Tufts Medicine in Lowell, MA seeks a Clinical Pharmacist to partner with physicians, nurses, and other health professionals to review and optimize medication orders, monitor drug therapies, and provide drug information across patient care areas.

You will lead clinical activities, educate students and staff, and participate in pharmacotherapy research, with a focus on safety, quality and efficiency in a hospital setting.

Qualifications

• Bachelor's degree in Pharmacy required.
• Pharmacist license required.
• One year of related experience required.
• Doctor of Pharmacy (PharmD) preferred.
• Pharmacy Residency training preferred.

Responsibilities

• Directs and coordinates the clinical activities of the assigned pharmacy service area.
• Reviews and interprets physician orders for indication, dose, allergies, interactions, and duplication; ensures appropriateness.
• Collaborates with physicians to resolve questions or irregularities regarding medication orders.
• Ensures medications are prepared and labeled accurately and that the department complies with state and federal regulations.
• Provides accurate drug information using pharmacology references and literature.
• May conduct medication history interviews and reconcile therapies for patients and families.
• Participates in medication education and documentation in progress notes; supports Medication Use Evaluation.
• Performs order entry and uses pharmacy computer systems accurately, including allergy and charging information.
• Participates in multidisciplinary meetings and discharge medication list reviews to improve reconciliation and optimization.
