Job summary

Advocate Health in Oshkosh, WI is seeking a licensed pharmacist to join our pharmacy team. This full-time role (40 hours per week) involves overseeing pharmaceutical care, counseling patients and caregivers, and applying evidence-based medication therapy management.

The position requires a PharmD or bachelor’s degree in pharmacy, licensure in the state, and immunization certification where applicable. Competitive pay from $54.90 to $82.35 per hour plus benefits.

Qualifications

• Bachelor’s or Doctorate degree in pharmacy from ACPE-accredited school of pharmacy or FPGEC certificate from NABP.
• Licensed in the state of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens based on biomedical literature.
• Provides counseling to patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in management of medical emergencies when applicable.
• Supports medication safety and quality improvement activities and reports events.
• Collaborates with healthcare team to manage formulary adherence and medication-use process.
